This event will be held:<\/strong><\/p>\n<p><strong>Time<\/strong><b> &#8211; 1pm for 1.30pm till 4.30pm approx.\u00a0 <\/b><b><\/b><\/p>\n<p><strong>Venue<\/strong> -Studio room, Bathgate Regal Community Theatre,\u00a0<b>24-34 North Bridge St, Bathgate, West Lothian, EH48 4PS<\/b><\/p>\n<p>&nbsp;<\/p>\n<p><b>The event is open to all independent providers of support for older people (residential and community based) in West Lothian.<\/b><\/p>\n<p><b> \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 <\/b><strong>\u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 PROGRAMME<\/strong><\/p>\n<p><strong>OBJECTIVES \u00a0<\/strong><\/p>\n<ul>\n<li><i>To receive an overview and update on &#8220;Reshaping Care for Older People\u201d\u00a0and proposals for\u00a0&#8220;Integration of Adult Health and Social Care&#8221;.\u00a0 <\/i><\/li>\n<li><i>Review and\u00a0consider the\u00a0greater focus on\u00a0outcomes in delivering\u00a0care, including development of a\u00a0personal outcomes approach. <\/i><\/li>\n<li><i>\u00a0Learn more about the changes to working practice and commissioning that will impact on your services. <\/i><\/li>\n<li><i>Provide an opportunity to hear more about the joint IRISS and Scottish Care project <b>Fit for the Future.<\/b><\/i><\/li>\n<\/ul>\n<p><strong>Session 1\u00a0<\/strong><\/p>\n<ul>\n<li>presentation and discussion on reshaping care for older people , Health and Social Care integration and Joint Strategic Commissioning\u2013 <b>led by David Pigott , Associate JIT<\/b><\/li>\n<\/ul>\n<p><b>\u00a0<\/b><strong>Session 2<\/strong><\/p>\n<ul>\n<li>An outcomes based approach to design and delivery of services including use of talking points &#8211;<b> led by Chris Bruce JIT Strategic Lead on Outcomes<\/b><\/li>\n<\/ul>\n<p><strong>\u00a0Session 3<\/strong><\/p>\n<ul>\n<li>The joint IRISS and Scottish Care project <b>Fit for the Future.\u00a0<\/b><\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>\u00a0Happy New Year from Fit for the Future!
