If you are interested in supporting\u00a0more person-centred care and integrated practice, then read on. We’ll share with you how we worked,\u00a0what we did and the tools we used. We also share what we achieved and what we learned\u00a0through evaluation and reflection.<\/p>\n
Keeping it Personal modelled new, more inclusive<\/strong> ways of working.<\/p>\n It \u00a0brought together people to work in equal partnership<\/strong> to improve care and support.<\/p>\n It is also a place-based<\/strong> approach. We worked in North West Glasgow to improve person-centred care for people with dementia and\u00a0the Motherwell area, for people with heart failure.<\/p>\n The project applied the principles of co-production<\/strong> and improvement methodology<\/strong>\u00a0to place people at the heart of improvement. You\u00a0can read more about this in our\u00a0About <\/a>section.<\/p>\n After recruiting our sites,\u00a0Iriss<\/a>\u00a0provided a framework of six facilitated sessions over a 12 month period with a focus on:<\/p>\n The detail of what we did, including tools used, is captured in the\u00a0the KiP:Dementia<\/a> and KiP: Heart Failure<\/a> blogs.<\/p>\n We set out to deliver\u00a0more person-centred care within the time-frame of the project and\u00a0increase peoples’ capacity and capability\u00a0to\u00a0work in more person-centred ways beyond this.<\/p>\n We aimed to:<\/p>\n Finally, if you like to read about what we achieved and our learning from the project, \u00a0you can learn more in the Evaluation<\/a> section<\/p>\n\n
